IVIG Treatment for Autoimmune Small Fiber Neuropathy

Study Purpose

This Phase II clinical trial is investigating whether Intravenous Immunoglobulin (IVIG) therapy can improve nerve fiber density and reduce pain in patients with autoimmune small fiber neuropathy. The study specifically focuses on patients with antibodies to TS-HDS, FGFR-3, or Plexin D1.

Key Study Details

  • Study Type: Randomized, double-blind, placebo-controlled Phase II trial
  • Participants: 20 patients (10 treatment, 10 placebo)
  • Treatment: Panzyga IVIG (2g/kg every 4 weeks for 24 weeks) vs. placebo
  • Primary Outcome: Improvement in nerve fiber density on skin biopsy after 6 months
  • Secondary Outcomes: Pain reduction, disability improvement, symptom severity

Who Can Participate?

Inclusion Criteria
  • Age 18 or older
  • Clinical and biopsy evidence of pure small fiber neuropathy
  • Elevated antibodies to TS-HDS-IgM, FGFR3-IgG, or Plexin-D1
  • Baseline pain score ≥ 4/10 on visual scale
  • Utah Early Neuropathy Scale score ≥ 4/10
  • Small Fiber Neuropathy Screening List score ≥ 11/84
Key Exclusion Criteria
  • Other known causes of small fiber neuropathy
  • Evidence of large fiber polyneuropathy on EMG/NCS
  • Severe heart, kidney, liver disease, or HIV infection
  • History of blood clots or hypersensitivity to immunoglobulin
  • Recent use of IgG products (within 6 months)

Why This Study Matters

Small fiber neuropathy affects an increasing number of patients, with 19%-34% of cases having an autoimmune cause. Current treatments like long-term steroids have significant side effects. This study could establish IVIG as an effective, safer treatment option for autoimmune SFN patients.
